How to justify your absence from classes/ exam?

Please be informed that starting from the new academic year (1st October 2025) new rules of presenting the medical excuse will come into force.

Please read carefully the new rules and make sure that you have checked your medical insurance policy and/ or chosen a medical provider.

The Medical University of Lodz requires that that a student’s absence from classes/exams be excused on the basis of a medical certificate. The University will only honor certificates issued according to the following rules/criteria (a sample attached):

1.Direct examination by a doctor. Determining the temporary inability of a student to participate in classes or exams due to illness or hospital/ other medical facility stay is possible only after a direct examination of the student’s health condition.
2.The medical certificate must include all the following elements:
·Date of issuance of the document;
·Doctor’s name and surname;
·Doctor’s stamp confirming the doctor’s qualifications and specialization;
·License number;
·Doctor’s handwritten signature (the electronic signatures will not be accepted);
·Student’s name and surname;
·Student’s PESEL number or passport number (if PESEL number is not assigned);
·Validity period of the medical certificate;
·Reason for issuing the certificate.
You have to get a paper version of the certificate, which you need to present to your coordinator at the dean’s office (after scheduling a meeting first).
NOTE! The Medical University of Lodz does not honor medical certificates:
·Issued on ZUS ZLA forms. This form is intended for individuals employed under a work contract and cannot be used for students to excuse their absence from classes or exams;
·Issued electronically through teleconsultation;
·Issued by prescription machines.
Please remember:
·An original paper certificate has to be delivered to your administrative coordinator within 5 working days after the reason for the absence began (i.e. first day of your illness).
·A certificate cannot be issued more than 3 days after the reason for the absence began (i.e. first day of your illness).
