Providing information to third party – authorization template

Taking into consideration the University obligation to protect students’ personal rights and personal data (GDPR), please be advised, that the University does not provide any information about students and the course of their studies to third parties and does not provide any documents concerning students without third-party authorization, prepared in writing and containing the student’s signature. Provided that the identity of the authorized person is verified positively, a person holding the above-mentioned authorization has the right to request the University to obtain relevant information or documents. In order to be verified, the third person should personally go to the University with an authorization and a photo ID.

In addition, we would like to inform you that due to the necessity of identity verification (GDPR), the University does not provide any information by phone.

The authorization should be prepared in written form and signed in front of the Dean’s Office employee or the notary; in case of any doubt, an employee of the Center may request official certification of the student’s signature.

We do not recognize the scans/copies of the authorization.

FOR GRADUATING STUDENTS

The documents confirming the course or completion of studies conducted in English in the fields of medicine and dental medicine, in particular: certificate of the course of studies, graduation diploma, diploma supplement, as well as their copies, are issued to the graduate or a person authorized by the graduate. The authorization should be prepared in written form and its original submitted to the Administrative Center for Studies in English; in case of any doubt, an employee of the Center may request official certification of the graduate’s signature.
